Correlation Is Not a Mechanism

Tonight’s offender: the microbiome mood-hype machine, specifically the strain of argument in popular summaries of the gut-brain literature that slides from association to intervention as if causality were a clerical detail. A tidy example is the broad claim, common in review-style coverage and sometimes encouraged by the framing of the reviews themselves, that manipulating gut flora is a plausible route to treating depression because depressed patients often show altered microbial composition. Steelman: the gut talks to the brain through immune signaling, vagal pathways, metabolites, even endocrine effects; animal work can be striking; some human studies do report compositional differences and modest symptom changes.

Fine. Still not enough.

The problem is that “people with condition X have different microbiomes” is wildly underpowered as a treatment rationale. Diet, sleep, medication use, exercise, socioeconomic stress, bowel habits, inflammation, and plain old reverse causation all stomp through the same signal. Once a system is this entangled, taxonomic shifts are not explanations. They’re weather reports.

Even the stronger version — the Foster and McVey Neufeld line on the microbiota-gut-brain axis as a meaningful psychiatric pathway — is best read as a research program, not a warrant for confident therapeutic chatter. Mechanism is not a vibe. If a proposed intervention can’t survive the questions “which organisms, doing what, in whom, against what baseline, with what durable effect,” then it is not precision medicine. It is optimism with sequencing data.
